When some medications are taken over a long period of time, the affect of the drug diminishes and the patient requires continually larger doses. This reaction to a medicine is called tolerance. Narcotics are among the drugs for which patients frequently develop such a tolerance.

Addiction refers to the habitual use of a drug. Addiction is a serious problem with many drugs. It may become when people are ill and receive narcotics or barbiturates over a prolonged period of time.

An awareness of which drugs are most likely to result in tolerance and addition is important. At the first indication that a drug is not acting as it did previously, or at the first indication or dependency upon a particular medication, the physician should be notified. Often alternative medicines can be administered as a preventive measure.

Guide to Assessing Nursing Needs
1. Does the patient have any allergies?
2. What medications have prescribed for the patient?
3. Why is the patient receiving these medications?
4. What observations should be the nurse make relative to the effect of these medications on the patient?
5. Are there specific nursing measures indicated because of the action of drugs contained in these medications?
6. How are the patient’s medications to be administered?
7. What precautions should be taken in administering these medications? Are there special precautions that should be taken because of the patient’s age, physical condition or mental state?
8. Do any of the medications require special precautionary measures in their administration?
9. Does the patient have learning needs relative to his medicinal therapy?
10. Does the patient or his family require specific skills or knowledge order to continue medicinal therapy at home?

Guide to Evaluating the Effectiveness of Nursing Action
1. Is the patient taking the medicines that have prescribed?
2. Have the medications been administered safely?
3. Does the patient exhibit signs or symptoms which indicate that the medications are effective (or ineffective).

1. Ordering of drugs from the hospital
• Only the head nurse and the charge-nurse are authorized to order drugs. All drugs that come up from the pharmacy are to be checked against the drug order notebook or the medication for initial orders. Only the medication-nurse does the receiving, checking and storing the medicines.
• The amount of all medicines ordered or received initially should be indicated at the lower left and corner of the medicine card.
